By April 1, 2012, every person in Maryland entering or living in a nursing home, assisted living facility, hospital, hospice, kidney dialysis unit, or hiring in-home care services is required to have one of these Medical Orders for Life Sustaining Treatment (MOLST) forms filled out and signed by a physician or nurse practitioner. A copy is to be given to the patient within 48 hours.
Reisterstown, MD - This beautiful 94 year old lady is leaving the nursing home where she has spent the last two years. If you look closely you can see the smile on her face as she goes to live with her daughter. Her daughter will be receiving lots of help with the care of her mother from the Medicaid Waiver Program. If your loved one has been in a nursing home for 30 days and meets the financial requirements, he/she is eligible to be considered for priority access to the Waiver. This lady started the process in December 2012.
